How to qualify

You qualify if you meet all of the following

  • You are on Medical Assistance (MA)

  • Be 18 years or older

  • Have a documented disability

  • Need assistance with communication, mobility, decision-making, or managing challenging behaviors

  • Be experiencing housing instability at risk for institutionalization

How we can help

We are here for you at every step in the process. From your initial consultation to helping you prepare for your move to sustaining your new home. We want to help you live your highest quality of and to embrace your independence.

  • Housing Transition Services:

    • Identify your budget, ideal location, and any important accommodations

    • Assist with applications for various programs and financial assistance you may qualify for

    • Search for homes that match your needs and preferences

    • Arrange tours of potential homes to ensure the right fit

    • Guide you through the application process and ensure you understand your lease

  • Housing Sustaining Services:

    • Coordinate your move and arrange moving assistance if needed

    • Provide education and training on the roles, rights, and responsibilities of tenants and property managers

    • Work with property managers or landlords to promote housing retention

    • Support you in understanding and maintaining/increasing income and benefits to retain housing

    • Coach you on developing and maintaining key relationships with property managers and neighbors

Step one

An assessment (MNChoices, Professional Statement of Need (PSN), or coordinated entry) is completed and your initial consultation takes place.

Step two

A housing coordinator will connect with you to develop a plan that aligns with your goals. Your needs are unique and so is your housing coordination plan.


Step three

We guide you every step of the way from searching for your next home and helping you with the application process to aiding you during the move in process.
