Relocation Service Coordination (RSC)

Relocation Service Coordination (RSC) is a program funded by Medical Assistance (MA) designed to support individuals residing in a Facility in locating housing within the community. It operates as Targeted Case Management (TCM), assessing both medical and social needs, and collaborates closely with the individual to facilitate a safe transition to their new residence.

Elate Care collaborates with multiple counties and managed care organizations to facilitate access to necessary services and supports. We develop a Person-Centered transition plan that prioritizes choice and ensures that comprehensive supports are established in the individual's new home!

How to Qualify

  • Eligible for Medical Assistance (MA)

  • Resides in an institution that qualifies for services at the time of service delivery

    • Hospital, including community behavioral health hospital (CBHH)

    • Nursing facility

    • Intermediate care facility for persons with developmental disabilities (ICF/DD)

    • Child and adolescent behavioral health services (CABHS)

    • Psychiatric residential treatment facility (PRTF)

    • Institute for mental disease (IMD)

  • Chooses to move into the community

  • Chooses to receive services

  • Targeted Case Management  benefit is not exhausted

    • Assisted Living

    • Independant Living

    • Adult Foster Care

    • Subsidized Housing

    • Housing Support (Formerly Group Residential Housing)

  • Creating and putting into action a relocation plan alongside the individual

  • Organize different income sources to afford housing

  • Aiding in accessing services (health, social, and financial services) and housing

    • Contact the appropriate county agency

    • Help individuals apply for MA

    • Access waiver services

  • Aligning efforts with the discharge planner at the institution

  • Aide in applying for different housing options even with negative renting history

  • Coordinate arrangements for moving day to ensure everything runs smoothly
